Individual Appointments
- Initial 45-60 minute consultation/assessment for $200
- 25 minute follow-up visits for $100
(Phone appointments may be substituted for face-to-face appointments)
Self-Pay Packages
- Initial 45 minute consultation/assessment
- Two 25 minute follow-up visits
- A $400 value for only $260
- Option: To determine your calorie needs, add an RMR test for an additional $50

(All visits must be used within 60 days)
*For Individual Appointments we are a provider for all major insurance companies, but coverage varies depending on your specific condition. All insurance users will be required to pay a $175 deposit which will be refunded entirely if your insurance covers. If this is not the case, this deposit can be easily applied to any self-pay package. We will submit insurance claims for your convenience.
**All packages are non-refundable and cannot be submitted to insurance. If you cancel an appointment with less than 24 hours notice or do not show up for an appointment, one visit from your package will be forfeited.
